Note: Waiver Forms are subject to change without notice, in line with changes to State and Federal Legislation as they occur.
Form types – what kind describes you best?
Casual Visitor: These are for people coming to Cliffhanger by themselves or with a few friends
Not for use with any form of Group Booking.

Groups. (Excluding Secondary Schools). These forms are for both adult groups, childrens groups, mixed adult and child groups such as clubs, scouts, social groups and also for Primary Schools or Special Needs Groups.
These forms are NOT for Secondary Schools or Birthdays.

All forms can be faxed to you and returned by fax on 9369 2400. Most forms can be emailed, please call us to arrange. Unfortunately not all staff have email access, so sometimes their can be a 1 day delay in emailing of forms.
Alternatively, all forms can be posted to you, it just takes a little longer.
